Critical violations are those factors leading to foodborne illness and must be corrected immediately. Noncritical violations relate to the maintenance of food operations and cleanliness.
1001 U.S. 412 West, Siloam Springs
Noncritical violations: Thermometer in the front milk reach-in is broken. An employee wearing fingernail polish is working on the finish line handling items from the fryer without wearing gloves. Door latch on the roast beef warming cabinet does not hold the door closed.
400 N. Eighth St., Suite A, Rogers
Noncritical violations: No certified food protection manager documentation available. Individually packaged containers of cheese dip and salsa not labeled.
Noncritical violations: No documentation of a certified food protection manager. Raw eggs in carton stored over containers of ready-to-eat food in the kitchen walk-in cooler.
Noncritical violations: Posted permit expired.
Critical violations: Both slicers had a buildup of debris.
Noncritical violations: Multiple nonfood contact surfaces have a buildup of dust and food residue.
Noncritical violations: Several dented cans at the seam on shelves for consumer sale. Raw packages of bacon on shelving over ready-to-eat franks.
1115 U.S. 412 West, Siloam Springs
Critical violations: Food in the walk-in cooler is at 45 degrees. Thermometer reading 50 degrees. Maintain food at 41 degrees or below.
Noncritical violations: Sanitizer concentration for wiping cloth bucket by the front register sink is not strong enough. Should be 200 ppm. Platforms cases of raw chicken are stored in the walk-in cooler are visibly dirty. Gap in floor/wall seal along the back wall of the center walk-in chicken cooler, grout is missing. Fill in as necessary to prevent standing water. Walk-in freezer door has ice buildup around it which prevents the door from closing all the way. These items are repeat violations during multiple inspections. These items need to be addressed and corrected permanently. Ice buildup on the condensation line in the freezer. Evidence of water dripping onto product cases. Condensation leak in the walk-in cooler. Facility has a sheet tray under the area where it is known to leak. Grout missing between floor tiles in various places throughout facility. Areas around the three-compartment sinks and the chicken breading stations are worse. Walk-in freezer door is not closing all the way without lifting it back on the hinge.
Critical violations: Raw shell eggs stored above ready-to-eat tomatoes in walk-in cooler. Front cold hold unit has multiple food items out of temperature: diced tomatoes at 47 degrees, cooked beef at 44 degrees, salsa at 45 degrees, and cooked chicken at 45 degrees. Large container of pico de gallo did not have a date marking in walk-in cooler. Dirty wiping cloths are stored on food prep surfaces.
Noncritical violations: Multiple employees working with food are not wearing an effective hair restraint.
Noncritical violations: The handwashing sink (knee pedal, no temperature adjusting handles) did not reach a temperature of 100 degrees.
Critical violations: Hand sink near grill leaks from waste pipe. Grease comes through floor drain when hand sink near grill is used.
1728 S. 46th St., Suite 101, Rogers
Critical violations: Employees eating and drinking at food prep table in food prep area.
Critical violations: Spray bottles not labeled with contents.
Noncritical violations: No certified food manager documentation available. Packaged juices not labeled.

1402 S.E. Shane Lane, Bentonville
Critical violations: Employee was eating food from plate in food preparation area. An employee was eating food and playing on a phone and went to prep/cook food without washing hands. No hand soap available at bar handwash sink. No paper towels available at bar handwash sink. Multiple food items were uncovered in first walk-in cooler. Food items in first cold hold unit were above 41 degrees: sour cream at 47 degrees, diced tomatoes at 46 degrees and guacamole at 46 degrees. Food items in vegetable/dairy walk-in cooler were also above 41 degrees: milk at 47 degrees, cheese at 46 degrees and beef at 44 degrees. Salsa in ice bath was at 70 degrees. Sanitizer in a wipe bucket was above 200 ppm chlorine. Multiple flies in back kitchen area where food was cooling. Bags of onions stored directly on walk-in cooler floor.
Critical violations: Minced garlic at 44 degrees in prep table and red sauce at 44 degrees in the prep refrigerator. Jelly at 46 degrees in the refrigerator.
Noncritical violations: No documentation of a certified food protection manager. Posted permit is expired.
Noncritical violations: No certified food manager certificate available at time of inspection.

Noncritical violations: Person in charge did not know where a copy of the food safety manager's certificate is located.

Critical violations: No sanitizer being dispensed into the dish machine. Sanitizer container is empty.

Noncritical violations: No documentation of a certified food protection manager. Posted permit expired June 30, 2022.
3355 U.S. 412 East, Siloam Springs
Critical violations: Packaged/bagged salad in the upright cooler. Cooler is not in operation at this time. It is not known why the salad was in the nonworking cooler. Discarded as needed. Foods on the make line are not at 41 degrees or below. The unit is not working. Facility says it is using ice as the alternative to keep food cold, but the ice had melted. Ice must be at or above the level of the food to be effective. Person in charge said maintenance is aware of the problem. Back-up items are stored in the walk-in cooler.
Noncritical violations: Gasket missing from the door for the fry bags. The door does not stay closed.
2811 Bella Vista Way, Bella Vista
Critical violations: Packaged burritos at 48 degrees in the hot pocket refrigerator.
Noncritical violations: No documentation of a certified food protection manager. No sanitizer at the three-compartment sink. Pizza at 114 degrees and sausage at 102 degrees. No test strips.
2260 Forest Hills Blvd., Bella Vista
Noncritical violations: No documentation of a certified food protection manager. The kitchen has no separate handwashing sink and employees are using the three-compartment sink to wash their hands. White refrigerator lacks a thermometer. Cartons of milk are in contact with water and ice that has built up inside of the milk box. No test strips.
Critical violations: Marinara sauce at 46 degrees in pizza prep table and chopped onions at 49 degrees in the pizza prep table refrigerator. Thermometer in pizza prep table refrigerator was at 48 degrees.
Noncritical violations: No documentation of a certified food protection manager. Four boxes of NyQuil that expired on 5/21, two boxes of Mucinex DM that expired 3/22 and three boxes of ZzzQuil expired on 10/21. Buildup of debris around the edges of the sandwich prep tables.

Critical violations: Three spray bottles of flying insect spray for residential use only observed in facility. At least a dozen dead cockroaches observed under equipment, on floor and in light fixtures. Three live cockroaches observed in buffet area. One live cockroach observed in cooking area. Critical violations: Soap bottles and sponges being stored in handwashing sink.

Critical violations: Bar dish machine was at 10-25 ppm when checked with chlorine test strip. Food items (cheese at 46 degrees and salsa at 45 degrees in ice bath). Wiping cloths stored in a sanitizer bucket with 0 ppm quat.
Noncritical violations: Permit posted but expired.
202 U.S. 412 West, Siloam Springs
Noncritical violations: One employee does have a certified food manager certificate. Need to provide a copy of the certificate within 10 business days. Bag of chicken strips sitting at room temperature to thaw.
2905 S. Walton Blvd., Suite 9, Bentonville
Critical violations: A bottle labeled sanitizer did not contain any sanitizer. Bottle labeled "bleach" is used for cutting boards and other food contact surfaces, concentration was over 200 ppm chlorine.
Noncritical violations: One employee prepping salad dressings was not wearing an effective hair restraint. Walls and fan in dish room have an accumulation of grease and food residue.
Critical violations: Men's restroom lacking handwash signage.
Noncritical violations: Cardboard boxes of food items being stored directly on floor of walk-in freezer. Posted permit expired.

Critical violations: Prepared foods in refrigerators do not have date marking.
Noncritical violations: No certified food protection manager on staff. No test strips available on site.
1702 S. Walton Blvd., Suite 12, Bentonville
Critical violations: 0 ppm concentration of chlorine from dish machine after multiple runs. Potato slicer (not in use) was visibly dirty from food residue. Food items (sliced cheese at 48 degrees, sausage at 46 degrees, chili at 48 degrees) in walk-in cooler were between 46-48 degrees.
Noncritical violations: No certified food safety manager. Ceiling has an accumulation of dust. Top of shelves in food preparation area has an accumulation of dust.
Critical violations: A box of bacon stored over ready-to-eat items in the prep table. Milkshake machine, slicer and dishware had a buildup of food and debris. Cheesecake at 44 degrees in the black refrigerator. Freezer at 40 degrees. A repairman has been called. Observed live and dead roaches. Packages of string cheese stored in water in the small glass refrigerator. Food debris, dead insects, trash and grease buildup on floors, walls, shelving, etc.
Noncritical violations: Personal drinks with lids and straws stored on food preparation tables and in refrigerator over food. Three bottles Robitussin expired on 10/21, Alka Seltzer 5/21, and NyQuil 5/21. Facility lacks refrigerator thermometers. Cups, packages and boxes for to-go items stored on the floor. The flooring is not smooth and lacks coving. Posted permit expired Sept. 30, 2021.
Gold Town Sushi & Korean BBQ
1100 S.E. 14th St., Bentonville
Critical violations: Employee is going from washing dishes to food preparation without washing hands. Crab salad in the cold hold unit at sushi area was between 58-62 degrees. A container of noodles in same cold hold unit was around 57 degrees. Crab salad was prepared yesterday and stored in cold hold unit. Menu lacks consumer advisory. Bag of carrots stored on walk-in cooler floor. Multiple containers/pots of food are stored on kitchen floor.
Noncritical violations: Facility lacks refrigerator ambient air thermometer in the two front sushi coolers. Knives and scissors are being stored in between two prep cold hold units in kitchen area.
Noncritical violations: Both handwashing sinks in kitchen and dish area leaking water from drain lines. Dump sink on server line clogged with standing water.
Noncritical violations: At least one employee who has supervisory and management responsibility and the authority to direct and control food preparation and service shall be a food protection manager who has shown proficiency of required information through passing a test that is part of an accredited program. Posted permit expired.
