Richland County restaurant inspections: Critical violations May 9-11

The following restaurant inspections with critical violations were conducted by Richland Public Health between May 9 and May 11:

• Joe & Mary's Market, Adm. Sturges Inc., 162 Sturges Ave., Mansfield, May 9. Water source and/or hot water capacity not sufficient (critical).  Three-compartment sink was unable to get a temperature of above 107 degrees after allowing the water to run for several minutes. Adjust above 110 degrees by reinspection.

• Dayspring Richland County Home, 3220 Olivesburg Road, Mansfield, May 9. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed residue on the blade of the can opener.

• Ontario Duchess #1249, 3675 Park Avenue West, Ontario, May 9. Person in charge (PIC) unable to demonstrate knowledge by having no critical violations (critical). Did not require employee(s) to report symptoms, diagnosed illness by a health care provider, or high risk situations as specified in rule (critical, corrected during inspection). Manager unable to locate employee illness agreements. Equipment food-contact surfaces or utensils are unclean (critical). Observed hot dog roller and tongs on retail floor to contain visible build up/residue. Please clean by May 11.

TCS foods not being hot held at the proper temperature (critical, corrected during inspection). Observed items in retail hot holding unit to not be at 135 degrees (chicken tenders 122 degrees, toasted pizza sub 122 degrees). PIC discarded food. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed multiple food and beverage items in walk-in cooler to be between 48-52 degrees. PIC discarded items. Raw shell eggs not maintained at 45 degrees or below (critical, corrected during inspection). Observed 12 dozen raw shell eggs in walk-in cooler to be at 52 degrees. PIC discarded eggs.

TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed multiple items in grab and go retail cooler to between 43-48 degrees. PIC discarded items. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed multiple TSC foods, open bags of pepperoni, roast beef, sausage patties, meatballs, pickled eggs, pizza sauce, & yogurt not date marked. Food discarded. Equipment and/or utensils improper construction (critical repeat, corrected during inspection).

Observed multiple clear non-food grade containers in kitchen and walk-in cooler to contain food (jelly, yogurt, granola). Opened food items not in original packaging or food grade bag must be stored in food grade container. PIC discarded non food grade containers and food contents. 

• Panera Bread #4780, 2070 Walker Lake Road, Mansfield, May 9. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Bagels uncovered at the drive-thru area. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Tomato slicer had visible build-up on the blades.

Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Cutting board for cilantro had visible stain on it. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Yogurt in tubes was observed at 51 degrees at the drive thru. PIC voluntarily destroyed 12 tubes. Presence of live insects (critical).

Observed live gnats at mop sink and at three compartment sink area. PIC stated a pest control company had just been called in. Correct by May 16. Insufficient air gap between the flood rim and the water supply inlet (critical). Insufficient air gap at the ice machine drain. Repair by May 16. Insufficient air gap between the flood rim and the water supply inlet (critical). Insufficient air gap at the dishwasher drain. Repair by May 16.

• Knights of Columbus, 250 Vernon Road, Shelby, May 10. Presence of live insects, rodents, and other pests (critical). Small rodent droppings observed in dry goods storage and cabinets by dry good storage. Please have copy of most recent pest control invoice and remove sign of activity by reinspection.

• Gionino's Pizzeria, 176 E. Main St. B, Lexington, May 10. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). Observed a container of chicken breading uncovered. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed residue on the blade of the can opener.

• Springmill Stem Elementary, 1200 Nestor Drive, Mansfield, May 10. Person in charge not ensuring proper cooling (critical, corrected during inspection). PIC was not monitoring time and food temperatures of leftover foods she was cooling. PIC stated she will no longer cool leftover food and instead discard leftover food. Food contact surfaces not easily cleanable (critical, corrected during inspection). Spautla observed rough around edges and no longer smooth and cleanable. PIC voluntarily discarded.

• Ontario Duchess #1249, 3675 Park Avenue West, Ontario, May 11. PIC unable to demonstrate knowledge by having no critical violations (critical). Equipment food-contact surfaces or utensils are unclean (critical repeat). Observed hot dog roller and tongs on retail floor to contain visible build up/residue. At time of inspection, hot dog roller not in use and manager states they do not use it. Clean by May 11.

Equipment and/or utensils improper construction (critical repeat, corrected during inspection). Observed croissants in non-food grade container in walk-in cooler. PIC placed croissants in food grade bags. Ensure all containers and bags in direct contact with food are food grade. 

• Apple Hill Orchards, 1175 Lexington Ontario Road, Mansfield, May 11. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). Observed a carton of raw duck eggs on a shelf above bottled drinks in the three-door cooler. PIC placed eggs on the bottom shelf inside the cooler. Working containers of poisonous or toxic materials not properly labeled (critical, corrected during inspection).

Observed a spray bottle of bleach water with an illegible label. PIC relabeled bottle with contents. No air gap or approved backflow prevention device on the plumbing system (critical). No backflow prevention device observed on the spigots connected to the spray hose in the cider room. Correct by May 18.
